We understand that not all illness and healthcare needs can be predicted, so please contact us as early in the day as possible for same-day appointments.
If you are a new patient and you have access to a printer, please download our new patient history form (historial de paciente) and new patient registration form (registro de pacientes) that can be filled out prior to your first appointment, which will reduce your time in the clinic.
If you are unable to fill out the forms in advance, please arrive at your appointment 10-15 minutes early.
