Need help paying for your medical care?

Affordable Care clinic is committed to providing accessible healthcare services. If you need assistance covering your medical costs at Affordable Care Clinic, we have a Patient Payment Assistance program (PPAP) which offers reduced fees and payment plan options for eligible patients.

ELIGIBILITY CRITERIA:

Patients must qualify based on household income level (Federal Poverty Level – FPL) and provide proof of income.

COVERED SERVICES:
  • Office visits (primary & acute care)
  • Basic in-office services & simple procedures
  • Specialty referrals
SERVICES NOT COVERED:
  • External Lab Services
  • Medications
PAYMENT EXPECTATIONS
  • Assistance reduces cost but does not eliminate the responsibility for payment
  • Minimum payment required at the time of visit:
    • $50 for discounted visits
    • cost of lab tests
    • cost of medications
  • Remaining balances must be paid within 90 days through an agreed upon payment plan.
